ANZBP Supervisor Application Form

Note the following definitions: 

Lead Supervisor

The supervisor(s) responsible for the overarching training, education and supervision.  Must be 5 years post ANZBP certification or be an OTP (ANZ) with at least 5 years of experience in perfusion post overseas qualification in perfusion.  Must hold a current CCP (ANZ) or OTP (ANZ).

Clinical Supervisor

Any supervisor responsible for the trainee in the clinical setting.  This person may differ on a case-by-case basis, depending on the clinical situation and capabilities of both the trainee and the clinical supervisor.  Must be minimum 2 years post ANZBP certification or be an OTP (ANZ) with at least 2 years of experience in perfusion post overseas qualification in perfusion.  Must hold a current CCP (ANZ) or OTP (ANZ) Assumes full responsibility for the case and be able to take over from the trainee as deemed necessary.

If no, you will need to email the following supplementary documents to abcp@anzcp.org, for consideration: 1. A letter of support from the lead supervisor at your unit, detailing your experience and value as a clinical supervisor 2. Overseas CCP (current).
Note: This submission does not automatically approve your supervisor status. You will receive a determination from the Board in due course.
